Can you produce one client's file within a month?
That is what the right of access asks for. If assembling a file means searching a drive, an inbox and a diary, the answer is no in practice even when it is yes in principle, and that is the first thing the software has to change.
Can you show what a note said before it was amended?
An amended note with no history is a note whose earlier version has gone. Version history is a records feature rather than a nicety, and it is worth asking about by name.
Decide what happens when a clinician leaves, before one does
The record belongs to the practice as controller; the clinician needs their own copy of what they wrote. A system that cannot express both will produce an argument at exactly the wrong moment.
