These sheets have no constants. Every number is worked from the figures you enter: the fields on your intake form, the required ones, the seconds a client spends on each, your new clients a month and how many never send it back, your session fee, your clinicians, sessions, working weeks, no-show rate and clinician split, your rent and software, and the minutes a note takes. The defaults are one three-clinician practice's worked example, not a benchmark this site publishes.

This is the SOAP notes sheet, and it prices the least visible cost in a therapy practice: the time notes take. Enter the clinicians writing notes, the sessions each does a week, the minutes one note takes, your working weeks, the share written after the day they belong to, the hourly cost of a clinician and the minutes a structured template saves. It returns the notes written a year, the hours and the money they cost, how many are written late, the hours a template would give back and what that is worth, and the hours per clinician and per week across the practice.
The hours, off the caseload
Three clinicians at 22 sessions a week over 46 weeks is 3,036 notes a year. At seven minutes each that is 354 hours, and at $62 an hour it is $21,960 of clinician time: more than most practices spend on software, premises insurance and CPD together.
The notes written late
30% on the worked example is 910 notes written after the day they belong to, usually at a weekend, from memory. That is a records quality problem before it is a time problem, and it is what a template and a five-minute end-of-session habit actually fix.

Is seven minutes realistic?
It is one practice's figure. Time five of your own notes end to end, including finding the file, and use that.
Does a template make notes worse?
Not if the fields are the ones the profession expects. It makes them faster and more consistent, and it stops the note that says session as before.
What about dictation?
It helps some clinicians a great deal and others not at all. Measure it the same way: time five notes with it and put the number in the sheet.
